We will use this, and work with you to create a care plan that suits the needs of your loved one. Facilitating support as and when they needed it.  

    Care plans are working documents, meaning that we consistently monitor them and amend them to reflect your loved one’s needs. We will always notify you of any changes and recommendations.  

    Every aspect of your loved one’s life will be considered for our care plan, from daily personal care routines to medical needs to social habits and hobbies. This enables us to encourage them to maintain as much independence as possible while keeping them engaged and active. 

    Yes, we do. We will make sure that our staff continues to provide the same kind and calibre of care whether your loved one is leaving the hospital, moving into a different care facility, or have been receiving care at home.  

    Open communication with all parties involved and close staff collaboration with NHS therapy teams when necessary are how we achieve this. 

    Absolutely! We understand that there are a number of reasons why your loved one might have dietary requirements, be it for religious reasons, health conditions or morals. Whatever the reason, we always accommodate dietary restrictions. Our chef will work with you and your loved one to create dishes that meet your needs and are just as delicious as what everyone else is having.  

    Family involvement is critical in the care of a loved one who has complex needs. We always want to include families when creating and maintaining care plans and believe that family involvement will allow us to provide the best possible care. We will make sure that there is an open line of communication between our staff and yourselves, so you are always up to date with the care and activities.
